Applying for a First Time Passport
First time applicants applying for a passport book, passport card, or both must complete Form DS-11: Application for a U.S. Passport. Additional items needed for application are listed below.
All photos submitted with applications for a US Passport must meet U.S. Department of State requirements. Photos can be taken at the City of Roseville Passport Office for $15.

All passport applicants must be present at the time of application, regardless of age. Visit the U.S Department of State website for more information on special circumstances:
Submit Proof of U.S. Citizenship
- Certificate of Citizenship
- Certificate of Naturalization
- Certified U.S. Birth Certificate
- Consular Report of Birth Abroad
Provide Proof of Identification
- Current Government Employee ID (city, state or federal)
- Current U.S Military or Military Dependent ID Card
- Previously issued U.S. Passport Book or Card
- U.S Certificate of Naturalization
- Valid Driver's License (clipped MN Driver's License with the yellow receipt is acceptable)
- Valid Foreign Passport (current)
Submit a Photo
All photos submitted with applications for a US Passport must meet U.S. Department of State requirements. Photos can be taken at the City of Roseville Passport Office for $15.
Submit Payment
- A personal check or money order is required to the U.S Department of State. This payment is for your passport book and/or card.
- A separate payment, paid in cash, by check from a Minnesota bank payable to City of Roseville, or by debit or credit card (MasterCard, Visa or Discover) is required. This payment is for your application processing and photo (if applicable)
