The Gathering

 

What

A day of stimulating activates for those experiencing memory loss and 5 hours of respite for their caregivers. The Gathering is offered through Lyngblomsten’s Community Services and is in collaborative partnerships with churches in the east metro area. In Roseville, Centennial United Methodist Church is a host site offered two times a month. 

The caregiver and their loved one will meet with one of the Gathering Social Workers for an assessment prior to enrollment. 

The Gathering relies on dedicated and trained volunteers who provide an engaging, fun and safe environment for people with memory loss.

Short-term Residential Memory Care

What

If you are a caregiver and you need a scheduled break of 3 - 30 days, this is an opportunity for someone requiring memory care to spend a short time in a specialized, residential memory care unit. This opportunity is dependent on availability, a reservation, an assessment and a physician’s orders.

This short-term memory care stay allows the caregiver to attend to his or her own mental and physical health.

Inpatient Respite



What

Short-term care is provided to an individual so that a family member or friend who is the patient’s caregiver can rest or take time off. This short-term memory care stay allows the caregiver to attend to his or her own mental and physical health.

  • Seven-day minimum
  • Book one month in advance.
  • Space based on bed availability.

What  
Participants enjoy group discussions, creative projects, music, exercise, games and fun. Caregivers can also have a break while the person they care for spends time with FamilyMeans staff and volunteers in a group setting.

Adult Day Services



Wilder's Adult Day Health Program



What

Wilder Adult Day Health serves adults with memory loss and disabilities. The program offers three different levels of activities using a small group and person-centered care model. All program participants can benefit from daily structure, meaningful activity and social engagement.

Memory loss services are separate from general day programming. Center specializes in caring for adults experiencing mild to severe memory loss. Providing specialty trained staff, dedicated designed space, programs focus on purposeful and engaging activities, small group model, support services and resources for caregivers.

What

The Roitenberg Family Adult Day Center provides socialization and programming that focuses on the mind, body and spirit! Participants should be medically stable and able to participate in the program, free from communicable disease, and able to move safely with the assistance of no more than one staff. Persons with dementia are mainstreamed.

Ebenezer Adult Day Program - Minneapolis 



What

Personalized care and activities are provided for adults age 55 and older during the day at an attractive and safe facility. People who need specialized programming due to memory or other cognitive losses are welcome. 

We work with each participant to meet his or her needs, whether it is to explore a new hobby or spend some quiet time alone.  We offer a variety of recreational and social activities, including:

  • Exercise classes
  • Trivia and current events
  • Discussion groups
  • Games – board and card
  • Wii interactive system
  • Bingo
  • Painting and crafts
  • Entertainment
  • Celebrations
The program fee includes continental breakfast, hot lunch and an afternoon snack, meeting one-third of the minimum nutritional daily requirements. A registered dietician plans all meals, which are prepared on-site.

Health and personal care services are available. Staff can assist with identifying transportation options.

Prospective clients are encouraged to take a tour of the facility, before deciding to enroll. Enrollment requires completion of enrollment papers, an updated Health Summary from a doctor, and authorization from a Case Manager for individuals on AC, EW or other county programs.
